This Week's Carenote - 06/17/09

We are an ALS Chapter and looking for a blank scheduling grid that we can give to our PALS caregivers, so whenever they have someone to come and fill in for them the person can then look at the grid and see what needs to be done at what time of day, et al, meds, exercise, feeding, etc.

Do you have anything like that or know where we could find one.

AARP, long term, life, health insurance, AAA might have medication grids and other checklists and forms on their website.

It sounds as though you are making a care plan for an individual. As I have worked in health care for around 15 years in both nursing home and assisted living settings... check out the ResiDex software at www.tenxsystems.com. Honestly I'm not a saleswoman - but they have a system that is exactly what you're looking for and they might be able to help you out.

www.donnayoung.org has all sorts of blank Calendar pages and other great household forms. use different colored sharpie markers to note meds, therapy, bath time, appointments etc Can use 1 page for AM activities and another for PM Activities etc. Hope this helps...
